Call the team and referral desk at (205) 555-0105 or fax the referral to the same number. We confirm that we have it and call the patient the same working day.

What helps us

The patient's name and phone, your contact details, the area of concern, the sport and any precautions you want us to follow.

What you get back

A written note after the first visit and when the plan of care changes, sent the way you prefer.

Athletic trainers and team physicians

With the athlete's permission, or a parent's, we send your athletic trainer the plan in writing and answer questions by phone.
